The House Committee on Veterans Affairs is holding a hearing in Washington about the Phoenix VA hospital.

Scheduling  problems and delays in health care delivery were revealed at an earlier hearing. Those revelations led to the resignation of Veterans Affairs secretary Eric Shinseki and the suspension of several local officials.

The first witness Monday was Dr. Debra Draper from the Government Accountability Office. She said the GAO investigation found a long list of problems.

"Weak and ambiguous polices which result in significant variation, confusion and increased risk of undesirable practices at the local level. Software systems that do no facilitate good practices, unclear staffing needs and allocation priorities. Inadequate oversight which relies largely on facility self-certification without independent verification," Draper said.

The VA inspector general told the committee that investigators will turn over any possible criminal violations to the Department of Justice for possible prosecution.
