A correctional officer was attacked Wednesday at a state prison. It’s the latest incident at a facility that’s been plagued with security issues.

The Arizona Department of Corrections (ADC) says an officer was stabbed by an inmate at the Lewis prison in Buckeye while escorting him to the medical building.

In a statement, ADC says the inmate used an improvised weapon to stab the officer in the chest.

The officer was transported to a local hospital with non-life threatening injuries.

The Lewis prison has come under scrutiny after leaked surveillance footage showed inmates were able to unlock cell doors and attack correctional officers. The governor has since launched an independent investigation into security at the prison.
